The Questionnaire: The 12 page survey instrument was divided into 9 sections: your job; your manager; time management; work, family and personal life; work arrangements; work environment; family; physical and mental health; and “information about you.” Virtually all of the scales used in the questionnaire are psychometrically sound measures that have been well-validated in other studies. A summary of the measures used, including the working definition of each of the variables, the source of the measure and its interpretation, can be found in the reports cited above.
